Can't Afford A Hypnosis Therapy Session?

Each month I give back and offer two free hypnosis therapy sessions for those that cannot afford a session, but could really use one. If you'd like to be considered for a free session, please apply below. 

After clicking the apply button, please fill out all fields and at the bottom attach a copy of last years taxes. I only need the 1040, 1040 EZ etc, I do not not need all pages. Please select the checkbox to be added to my VIP list (required).
